Breast Augmentation
Breast augmentation means the insertion of a prosthesis in a surgically created pocket under the mammary gland or under the pectoral muscle. This ultimately depends on the initial appearance of the breast .There are different ways of introduction of the implants: inframammary, periareolar, around the navel or at the armpit level. The most used ones are the first two of them.
Also, there are different types of breast implants, depending on several characteristics, namely: shape (round or anatomical, "drop form"), volume, shell prosthesis (smooth or rough) and content (silicone cohesive gel silicone, saline, hydrogel). The implant can be placed under the mammary gland (in cases with sufficient glandular mass to mask or when there is a moderate degree of ptosis), or under the pectoral muscle (when the breast is very small). Depending on the patient's desire and the original shape of the breast, the surgeon must make the best choice for every case.
If the patient has a moderate breast ptosis, first it may be required a lift surgery, which ultimately shrinks the skin envelope before implantation. Also in case of a breast abnormality (mammary agenesis, a failure to breast development), the first surgical maneuver will suppose the introduction of a prosthesis. The gradual expander inflating will progressively create the pocket that will host the final implant.
Usually, the surgical intervention is performed under general anesthesia and the patient will remain under surveillance in the clinic for the first 24 hours after the operation. Subsequently, to gain and maintain a beautiful shape of the breasts, the patient must wear a bra constantly for about 4-6 weeks.
The only real problem is that the implant is a foreign body. The body usually accepts and tolerates this, but try to isolate it by wrapping it in a fibrous tissue (forming the so-called "capsule"). Over time, this capsule, as any fibrous tissue, has a tendency to constriction that can raise the prosthesis. At this moment, the breast becomes globular and painful by the appearance of "capsular contracture" and may require the replacement of the implant and fibrous capsule dissolution. That depends on the reactivity of each body and the quality of the implants.
In order to decrease as much as possible the body's response to the healing, we prefer the implants with textured or covered with a foam micropolyurethanic surface. Studies worldwide have shown that the average lifespan of the implants is 15-20 years.
Another question is that if the insertion of a breast prostheses alters the ability of a woman to breastfeed or not. Since they are inserted under the muscle or under the gland, they do not interfere with the latter, allowing a normal feeding pattern.
